Occupational Therapist - Independent Contractor – Home Program

Were looking for
a part-time Occupational Therapist
to support patients transitioning from hospital to home in a community-based setting. In this
independent contractor
role, youll provide in-home assessments, recommend assistive devices and adaptations, and help clients improve function, independence, and quality of life. As part of an integrated care team, you’ll focus on population health and chronic disease management, empowering patients to regain control of their daily activities post-discharge. This role requires
a minimum of 10 home visits per week , and could be ideal for someone looking to supplement another OT position.
What You’ll Do:
Conduct in-home functional, cognitive, and environmental assessments
Develop personalized care plans and SMART goals
Recommend and fit assistive devices and mobility aids
Educate families/caregivers on safety, mobility, and daily living support
Collaborate with care teams, community partners, and rehab assistants
Submit timely reports and maintain accurate clinical documentation
What You Bring:
Registered Occupational Therapist (COTO)
Previous experience with home visits for a minimum of 1 year
Experience in home care and working with interdisciplinary teams
Strong knowledge of ADLs, assistive devices, and chronic condition management
ADP authorizer (or eligible) preferred
Valid driver’s license and reliable vehicle
Additional language skills, particularly French, are a plus
