Vancouver Coastal Health is looking for a Substance Use Outreach Clinician to join the Aftercare Clinicians team at Heiltsuk Hospital in Bella Bella, BC. As a Substance Use Outreach Clinician with VCH you will Work as a member of an interdisciplinary team to provide a range of mental health and substance use supports. Use an outreach model that is strengths-based and inclusive of harm reduction approaches, this position will support client goals including detox/substance use stabilization, treatment and recovery programming, as well as those in the pre-contemplative stage, in addition to supporting wellness initiatives. Provide harm reduction education, group facilitation across the transtheoretical model of change, and case management toward and between treatment initiatives are part of the role. Establish and maintains relationships with clients. Collaborates with multidisciplinary team and other community resources, liaises and consults with internal/external physicians, governmental and community social service, health and mental health agencies to facilitate clients’ wellness journeys. Participates in committees and meetings, engages in on-going team planning, development and quality improvement activities, and maintains and updates own knowledge. Provides direct clinical services in the form of immediate assessment, intervention, referral and support to clients in urgent situations. Provides crisis intervention-stabilization as required and collaborates with colleagues and other caregivers in the identification and resolution of client care issues, ethical dilemmas, coordination and integration of care. Acts as client advocate, supports client’ recovery goals including building recovery capital, develops, coordinates and delivers evidence based treatment/interventions. About Bella Bella, BCBella Bella is a beautiful island community off the central coast of British Columbia, 690 km north of Vancouver. Education & Experience Master’s Degree in a health science discipline from a recognized program. Two (2) years’ recent experience practicing in community based or Indigenous-led or First Nations organizations working with individuals with substance use and mental health presentations; or an equivalent combination of education, training and experience. Valid B. C. driver's license. Pursuant to Section 41 of the British Columbia Human Rights Code, preference will be given to candidates who identify as Indigenous including First Nations, Inuit, and/or Metis. Knowledge, Skills & Abilities Demonstrate knowledge of substance-use recovery services in the community and across British Columbia. Demonstrate Knowledge of health inequities and discrimination faced by designated client population. Demonstrate knowledge of the principles of harm reduction in a health care setting. Demonstrate knowledge of strengths based, client centered approach and ability to incorporate into practice. Demonstrate knowledge of Indigenous Cultural Safety. Demonstrate knowledge of traditional healing practices. Demonstrate knowledge of trauma-informed practice. Demonstrate knowledge of substance-use recovery model. Knowledge of the principles of recovery in mental illness. Knowledge of Psychosocial Rehabilitation methodology. Broad knowledge of clinical theory & counselling skills (individual and group based). Broad knowledge of mental illness and treatment. Broad knowledge of substance use and addictions treatment. Broad knowledge of psychopharmacology (indications and side-effects). Broad knowledge of psychoeducation principles and methods. Broad knowledge of other facilities and community resources. Broad knowledge of legislation and government policies and procedure that relate to client care including the BC Mental Health Act. Knowledge of DSM IV Diagnostic Classification System, mental status examination and case formulation. Demonstrated ability to conduct mental health and addictions assessments, and provide treatment planning and crisis intervention. Demonstrated ability to communicate effectively, both verbally and in writing, with colleagues, clients and their families, students and service providers in health and social services. Demonstrated de-escalation and conflict resolution skills. Ability to manage aggressive and suicidal behavior. Ability to work with a culturally diverse population. Ability to conduct a risk assessment and respond appropriately. Demonstrated ability to establish workload priorities. Demonstrated ability to work both independently and collaboratively as a member of an interdisciplinary team. Demonstrated ability to adjust to new or unexpected events. Demonstrated ability to problem solve. Ability to operate related equipment. Physical ability to perform the duties of the position. Ability to operate a computerized client care information system and word processing, spreadsheet, Internet and email software.
